Issuing new ration card is an on-going process of Civil Supplies Department that function under State Govt. After implementing NFSA system with the advantage of technology, the entire system has been revised and brought to networked digital version. Hence most of the application can be submitted online thus visiting TSO / CRO can be avoided by applicant.
Though online facility is more convenient, people may want to submit the application off-line too because of their own reasons.Those who wish to apply offline, here is the application form for applying new ration card under new NFSA system.
This New Ration Card Application Form is printed in Malayalam, has 4 pages in which first page required basic information of the applicant including photo. It also lists name of supporting documents as given below:
- Resident's Certificate
- Birth Certificate
- Transfer certificate from another Taluk
- Transfer certificate of a member from another Taluk
- Transfer certificate from another State
- Non-renewal certificate (existing ration card but expired validity)
- Non-inclusion certificate (Not included in the renewed ration card)
- No Objection Certificate
- Cancelled Employment Visa
- Retirement Certificate from Defense Services
- Income Certificate
The second page seeks information about members whom to be included in the new ration card. It includes, Name (both in Malayalam and English), date of birth, relation ship with applicant, Gender, employment, monthly income, whether NRK / NRI, whether Annapoorna, and election ID.
The third page which is an extension of second page seeks more details about individuals such as Aadhaar details, whether paying income tax, whether member of Aashraya project, whether differently abled (disability in physical / mental), whether autism affected, whether destitute and poor, whether affected by deadly decease etc.
The fourth page required mainly financial status including bank account and property details.
