Civil Supplies Department, Govt. of Kerala issued a PDF application format for Inclusion of New Members in Existing Ration Card. This two page application form has to be downloaded and printed by applicant. After taking printout, check what are the documents applicant to be attached as the supporting document. Prior filling the application collect all those documents from their issuing offices.
The table in the form has listed following documents in which applicant to select from and produce along with the application:
- Birth Certificate
- Ration Card Transferring certificate from another Taluk
- Members transferring from another Taluk
- Members transferring from another State
- Non-Renewal Certificate
- Non-Inclusion Certificate
- No-Objection Certificate for Issuing Ration Card number
- Cancelled Employment Visa
- Retirement Certificate from Defense Services
- Income Certificate
- Certificate issued by MLA / President of Grama Panchayat / Block Panchayat / Zilla Panchayat / Municipal Chairman / Corporation Mayer
Other Supporting documents to be attached with the application form are:
- Copy of Aadhaar Letter
- Income tax
- Disability Certificate
